Live Case Study Call – November 10 2022 (Hashimoto’s Thyroiditis)

Case Overview:

  • Ultimate goal: understand what is going on with her thyroid and GI issues
  • 41yo
  • Mom of 2
  • Weight: 54k, after Covid, gained 7k – difficult to lose; wants to get back to 52.5kg
  • Felt vibrant/well: summer 2022
  • Concern: parents are aging
  • Two things: less anxiety, more active
  • No serious illness/hospitalization
  • Sleeps well (6-7 hours), 10/11pm – 630am
  • Cycle: 6 days of flow; regular; bit of bloating but not overly symptomatic; was taking birth control in her 20s (for contraception); no vagina infections/reproductive concerns
  • GI Symptoms: constipation, diarrhea, gas, bloating (started in her 20s)
  • Used to take a lot of laxatives; gas was really bad in her 20s
  • Supplements & Medications: thyroxine (87.5g/day), liposomal Vit C/D, omega-3s, zinc 30mg, selenium, probiotic
  • ABX history, unknown
  • Toxin Exposure: mercury fillings but they have been removed
  • Dental health during childhood: not ideal; currently has 3 missing teeth (bridge/implant)
  • 28: overactive thyroid; was told she has Graves and that she would eventually have an underactive thyroid; recent doc questioned if he actually has Graves or just the initial stages of Hashimotos (body speeds up when it senses a need); body fluctuates
  • Thyroid went underactive in her 30s. 
  • Energy = okay
  • Seeing a therapist; studying to be a health coach
  • Recent injury holding her back from physical activity
  • Mostly whole foods; tries to make healthier choices
  • Cravings: sugar, coffee

Symptoms:

  • Hives, allergic breakouts, dry-skin, hair-loss
  • Frequently feeling cold, excessive sweating
  • GI symptoms
  • Memory loss
  • Anxiety, anger, irritability, aggressiveness, mood swings
  • Low libido
  • Genital itch; discharge

WHAT’S GOING ON?

  • Is coffee playing a role in her anxiety and irregular heartbeat?
  • Sugar cravings at night; could it be the result of not having a well-rounded diet throughout the day (i.e., not enough fats/protein)?
  • What can lead to cravings?
    • Insulin resistance or some blood sugar issue?
    • Yeast overgrowth
  • Birth Control effects: depletes B vitamins; indirectly leads to low bile which can impact digestive function

LABS:

  • MCV: size of red blood cells; something is causing: low b9, b12 or vit C
    • Birth control depletes B vitamins
  • Lymphocytes: high; possibly a viral infection when labs were done
  • Calcium: lower end; potentially due to low intake
  • Magnesium (serum): uncertain since it is serum levels not tissue levels
  • B12: blood levels okay but uncertain if levels in the cells are okay (need RBC B12); if RBC B12 not getting into cell it could an issue of mercury toxicity
  • Thyroid Antibodies: high
  • TSH: less than 1; might be too much 
  • Free T4: slightly above upper half; medication working
  • Free T3: slightly love than half; may not be converting well
  • Cortisol: low; how does relate to thyroid?  Thyroid/Adrenal connection; cortisol affects hormone receptors; need to understand why cortisol so low?

RECOMMENDATIONS:

  • Check: RBC B12, Ceruloplasmin, Zinc, Copper, Vitamin A; Liver levels
  • OAT test: deeper look into systemic inflammation 
  • Talk to Dr to add a compounded T3 to medication
  • Support brain health: ADB5-PLUS
  • Adrenal support: grounding, journaling, mindset work, ice baths; induce feeling everyday
  • 5-HTP + B6 for motility and anxiety support (Motil-Pro by Pure Encapsulations)
  • Focus on low FODMAP foods
  • B Complex then switch to B Complex Number 6
  • Full liver panel; DUTCH test
  • H Pylori test
  • Gut healing: GI Revive; remove food sensitivities; binder to help remove heavy metals; sauna/sweating
  • Strengthen Immunity: immunoglobulins (gut immunity)
  • Digestive bitters
  • Dandelion Root: replace coffee; support biliary function
  • Amino acids to support sugar cravings
  • Increase protein intake
  • GI healing to ensure proper digestion and absorption
  • Consumption of seaweed; kelp (iodine)
  • After initial healing, broad spectrum cleanse
